STEP 3:

Accomplish the Test Service Agreement Form (TSAF) then send a copy to us.

If your school is located in Metro Manila, Cavite, Laguna, Batangas, Rizal and Bulacan,
contact your Testing Coordinator at (02) 8804-2145 or 8895-8236  or email tss@cem-inc.org.ph
Otherwise, contact the CEM Center Coordinator nearest you.

 

Membership validity covers the period of the current fiscal year from June 2022 to May 2023.
